Medical Summary 05/10/17 DVM Intake Exam Estimated age: 1 yr. Microchip noted on Intake? No. History : Stray – no medical concerns noted. Subjective: Observed Behavior – Fearful, became fractious when handled, fled and would not allow handling. Sedated with Telazol 0.15 ml for exam and treatments. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BAR prior to sedation. mm=pink, moist, CRT,2 s. HR=220, RR=24. BCS=5/9 EENT: Eyes clear, ears clean, no nasal discharge noted. ORAL: All adult teeth present, no oral lesions. PLN: No enlargements noted H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Male, both testicles palpable. MSI: Ambulatory x 4 prior to sedation, no palpable abnormalities. Skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities A: Apparently healthy young adult cat. P: OK for neuter surgery. Excellent prognosis.
