SAFE 10/4/17 **POSSIBLY BLIND** Sweet Hazel was brought in as a stray – she is about 10 yrs old. Hazel is possibly blind. She was emaciated, and severely matted. Needs further medical evaluation and rescue asap!!

History : Stray Subjective: Observed Behavior – very easy to examine, even though not visual Evidence of Cruelty seen – severely matted Evidence of Trauma seen – none obvious Objective T = 98.1 P = 180 R = wnl BCS 2/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: minimal tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: l mg 2 lat mass 1/2 golf ball size MSI: Ambulatory x 4, skin free of parasites,severely matted trunk scapula distally CNS: mentation appropriate.
