History : Stray
Subjective:
Observed Behavior – BARH. Allows handling, attention seeking
Evidence of Cruelty seen – No
Evidence of Trauma seen – No
Objective
P = WNL R = WNL BCS 4/9
EENT: Eyes clear, ears clean, serous nasal discharge, sounds congested
Oral Exam: Very mild dental staining, pink mm
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Male
MSI: Ambulatory x 4, skin free of parasites, no masses noted, areas of alopecia at distal lateral forelimbs and hocks — woods lamp neg
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: URI, alopecia – R/O overgrooming/stress
Plan: Continue to monitor while at BACC. Rec doxycycline 0.8ml PO q24 x 10 days
Prognosis: Excellent
