DVM Intake Exam

Microchip noted on Intake? no
History : Field picked kittens up in box in Queens. Mother ran away.
Subjective: Kittens are BARH.
Observed Behavior – Kittens hissed and ran to back of cage when approached, but allowed all handling once out of cage. Startled by quick movements or noises, but recovers quickly. Tendency toward flight not fight. Allows all body parts to be touched.
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
Objective
T = NA P = wnl R = wnl BCS 4-5/9
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: healthy deciduous dentition; no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: wnl
MSI: Ambulatory x 4, flea dirt++, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: not performed
Assessment: Underage/Underweight, Flea dirt, FIV+
Plan: FIV+, recommend retesting at 7 mos. Seek rescue placement.
Prognosis:
Good
SURGERY: Temporary waiver due to underage/underweight
