04/05/17 10:35 Upon intake Frederick has a loose and relaxed body posture. He rubbed his head against cousnelor and purred softly. Cousnelor was able to scan (negative) and collar. He walked out the carrier and solicited attention from counselor; he was photographed and walked back into the carrier. Finder had Frederick in the home for a week. Frederick was fed wet and dry food Friskies brand and was fed twice a day. Frederick spent time with two adults, five cats and a Chihuahua. Finder stated he was friendly and sought attention from the adults in the home. Frederick was relaxed and tolerant around the five other cats and the dog as well. Frederick was provided an open litterbox and Arm and Hammer litter.

KNOWN HISTORY: Frederick was brought in as a stray, so we cannot speak to his behavior in his previous home. MEDICAL BEHAVIOR: Quiet and seems friendly in carrier and when first out, becomes progressively more fractious with handling including hissing, growling, and attempts to bite EVALUATION: Cage Condition: Cage slightly re-arranged Reaction to assessor: Frederick meows then slowly comes forward seeking attention. Reaction when softly spoken to: Frederick paces back and forth, brushing his body up against the kennel door. Reaction to cage door opening: Frederick is calm and relaxed. Reaction to touch: Frederick sniffs the assessor’s hand and accepts petting all over his body. His starts to quiver when rubbed and he slowly leans in, giving soft head butts. Reaction to being picked up: Frederick was calm, relaxed and allowed all handling. His tail thrashes around and he meows softly to be put down. ACTIVITY LEVEL: Lively VOCAL: Somewhat chatty CHARACTER TYPE: People Oriented, Sweet, Affectionate Please note that Frederick is being treated for an upper respiratory infection at the time of the behavior assessment. This condition may affect some of the behaviors shown during evaluation. BEHAVIOR SUMMARY: Average Frederick interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

DVM Intake Exam Findings History: Surrenderer is TNR rep who trapped cat and took him to be neutered last week in hopes to release him. Since then she has noticed he is underweight and having difficulties eating; there may be something wrong with his mouth. Subjective Observed Behavior – quiet and seems friendly in carrier and when first out, becomes progressively more fractious with handling including hissing, growling, and attempts to bite Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 130 R = sniff BCS 2/5 (3-4/9) EENT: Mild-moderate epiphora and chemosis OU; Large amount dark and sandy debris AU, very pruritic when cleaning; serous nasal discharge and audible congestion noted; sneezing multiple times Oral Exam: cat is extremely painful around mouth; has ptyalism and halitosis; brief oral exam revealed very inflammed gingiva, especially at back of mouth PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Seems slightly distended, cat growls on palpation, no masses palpated U/G: recently neutered, peri-preputial area shaved, no erythema or edema, dark scabbing over scrotum MSI: Ambulatory x 4, no masses noted, hair coat slightly unkempt CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: URI, Stomatitis, Possible ear mites, Normal post-op neuter, FIV+ Medical Behavior Status YELLOW Medical Status 5C Asilomar Status UNH&UNTREA Fast Track? No Plan Already neutered. Recommend rescue placement only due to FIV status and stomatitis. Starting on buprenorphine BID x 3d and Amoxi/Clav liq. BID x 7d. Reassess oral pain on day 3 of buprenorphine and reassess stomatitis and URI on day 7 of abx.
