DVM Intake Exam
Estimated age:2-3 years
Microchip noted on Intake? negative
History : stray, found on street with fleas and finder bathed in oils. Cat was covered in oils on intake
Subjective: QARH. Covered in oils and dumpy.
Observed Behavior -very sweet but also dumpy from the oils. Perked up after the bath. Did well for all medical procedures.
Evidence of Cruelty seen -no
Evidence of Trauma seen -no

EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: cleft lip, clean adult dentition, no oral ulcers
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: FI, no spay scar noted, no MGTs, no vulvar d/c
MSI: Ambulatory x 4, live fleas noted, no masses noted, diffuse scabby dermatitis, open sore on right caudal hock
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment:
Lethargic
Covered in oils
Scabby dermatitis r/o FAD vs other allergies vs ringworm
Open sore on right caudal hock
Cleft lip
Prognosis: good depending on how she responds to surgery
Plan:
Bathe in warm water
Convenia 8mg/kg SQ once
Start clavamox 13.75mg/kg PO BID x10d
LRS 100ml SQ SID x3d
Wood’s lamp-negative
Recheck tomorrow
Scrub hock with chlorhex-reassess tomorrow
