GONE 1/1/18 *SWEET DIABETIC SENIOR KITTY EGYPT NEEDS HOSPITALIZATION ASAP* Egypt is a very friendly, talkative kitty who’s been asking for attention nonstop since his arrival at the Brooklyn Care Center yesterday. He’s emaciated and severely diabetic and needs to go to a vet hospital ASAP.

Subjective: BAR, ~6-7% dehydrated. Very vocal. Ate all food in bowls. Normal bm and u in cage. Ataxic and weak but alert. No c/s/v/d. Reviewed bloodwork from yesterday (see below) and is diabetic.

Oral Exam: adult dentition with moderate dental disease, pale mm
PLN: No enlargements noted

ABD: Non painful, no masses palpated, doughy
U/G: MN
MSI: Ambulatory x 4 but weak with dropped hocks, skin free of parasites, dull unkempt haircoat, emaciated, no masses noted
CNS: Mentation appropriate, ataxic

Geriatric with underlying condition (hyper T4 vs chronic renal disease vs FIP vs neoplasia vs other)
Prognosis: Poor
Plan:
CBC 12/30: monocytosis 0.73 (0.05-0.67), neutrophilia 13.60 (1.48-10.29), severe non-regenerative anemia 19.4 (30.3-52.3), thrombocytopenia 119 (151-600)
Chemistry 12/30: elevated ALT 226 (12-130), elevated BUN 56 (16-36), hypocalcemia 6.6 (7.8-11.3), hyperglycemia >686 (71-159)
T4 12/30: wnl (0.9)
BG 12/31 8:30 am too high to read
Start on 3U NPH SQ-will adjust dose accordingly

CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment: Geriatric with underlying condition (hyper T4 vs chronic renal disease vs FIP vs neoplasia vs other)
Prognosis: poor
Plan: Blood for CBC/CHEM/T4 submitted at time of exam. Eating with good appetite — continued monitoring while at BACC. If no placement recommend EHR.
