Medical Summary 9/28/16 Microchip scan neg. O: BARH. mm=lt pink, moist. Allows all handling. ORAL: Mod. calculus, gingivitis. EENT: No oculonasal discharge. Eyes sunken OU. H/L: Tachycardia – HR= 250 bpm. No murmurs ausculted. ABD: Palpation WNL. UG: Neutered male. MS: Severe, generalized muscle wasting. Amb x 4. INTEG: Mats throughout haircoat. A: Emaciated geriatric cat. DDX: Renal disease, hyperthyroid, liver disease, neoplasia, other. Dental disease, matting. P: Shave matstomorrow. Monitor appetite and weight while at BACC. Consider blood tests if not doing well. Rec. placement – will need further workup and treatment. Fair prognosis. FVRCP, Rabies vaccines, applied Activyl, Pyrantel 0.45 ml PO.
