Medical Summary 06/01/17 DVM Intake Exam Estimated age: 2.5 weeks Microchip noted on Intake? No. History : Stray kitten, no medical concerns noted. Subjective: Observed Behavior – Very active, vocal, curious about surroundings. Allowed all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR>240, RR=28, BCS=5/9. ORAL: No teeth erupted yet. No oral lesions. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Intact male. MS: Ambulatory x 4, crawling, no lameness. INTEG: Full, clean haircoat. No skin lesions or fleas. CNS: Mentation appropriate, no signs of neurologic abnormalities. A: Apparently healthy kitten. P: Temporary waiver from neuter surgery due to age. Needs placement. Excellent prognosis.
