Medical Summary DVM Intake Exam Intact male Estimated age: 5 weeks Microchip noted on Intake? negative History : Stray kitten. Having diarrhea. Subjective: Intact male kitten with diarrhea. Observed Behavior – Playful. Friendry. Not trying to flee. Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective T = 101.5 F P = 200 bpm BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: NAF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. Distended abdomen U/G: Both testicles present MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Not performed Assessment Diarrhea- parasites vs visrus vs food -related Distended abdomen- Parasites vs virus vs food- related Plan Parvo snap test- negative FVRCP vaccine given sq Felv/FIV- negative for both Metronidazole suspension ( 50 mg/ml) – 0.13 ml BID PO for 5 days Recheck weigh and condition in 2 days. Prognosis: good Temporary waiver due to diarrhea and underweight.
