Spay-Neuter Waiver Documentation
Vet Notes: 2:12 PM
[Spay/Neuter Waiver – Age]
It is the policy of ACC not to perform surgery on any animal over the age of 8-10 years due to the higher risks incurred in a shelter setting. The veterinarian is hereby issuing a permanent spay/neuter waiver, from the spay/neuter requirements of the City of NY due to the estimated age of this animal. ACC does recommend you consult with your veterinarian to determine if surgical sterilization is appropriate.

KNOWN HISTORY:
Daisy was brought in without information on her behavior history or tendencies in a home environment.
MEDICAL BEHAVIOR:
03/25/18
A bit nervous. During oral exam, p opened mouth, as if trying to bite, once. Well behaved during remainder of exam.
ENRICHMENT NOTES:
03/25/18
Lying behind litterbox, body pressed up against kennel wall. Tolerates petting along head and body while shifting weight and turning head to watch the bear-claw. Eyes are wide and ears are flat. Nervous, needs more time to adjust.
03/26/18
Crouched in back of cubby, body pressed up against kennel wall. She makes eye contact as I open the kennel door and flattens her ears. Tolerates slow and gentle petting along the head and body while staring straight ahead and beginning to rapid breathe. Not interested in treats at the moment – possibly too focused on me. Nervous and unsure, needs time to adjust.
Cage Condition:
Cage is neat
Reaction to assessor:
Daisy was tense and curled up in the back of the cubby, with her tail wrapped around her body.
Reaction when softly spoken to:
Daisy starts breathing rapidly and turns her head towards the wall to avoid eye contact.
Reaction to cage door opening:
Daisy lip licks and keeps her eyes focused on the assessor.
Reaction to touch:
Daisy shrinks her body, her ears bend flat and she hisses when the assessor extends his hand out. She flinches when touched and tolerates petting along her body with a slow approach. Her body feels tense and she seems very uncomfortable with extensive handling.
ACTIVITY LEVEL:
Laid back
VOCAL:
Quiet
CHARACTER TYPE:
Timid
Independent
POTENTIAL CHALLENGES:
Fearful
Potential challenges comments:
Daisy has displayed fearful behavior during their stay in the care center and may dislike certain types of handling. A fearful cat will feel more relaxed when given options, so provide her with the chance to move closer, investigate, or interact with you. Be sure to offer incentive such as treats or play time whenever the cat makes a small positive step. Please speak to an adoption counselor for additional information on methods to desensitize your cat to their fear stimulus.
BEHAVIOR DETERMINATION:
Experienced, adult only
Behavior Asilomar
TM – Treatable-Manageable
RECOMMENDATIONS:
None
BEHAVIOR SUMMARY:
Daisy tolerates attention and petting but may be fearful or stressed in the shelter, and may be intimidated by small children. She may be a little more independent, and may need time to warm up to her new home.
Due to the behaviors seen in the care center, we feel that this cat will do best in an experienced, adult only home.
