Medical Summary DVM Intake Exam Findings History: Owner surrender; owners 3 wo daughter is allergic to cat Subjective Observed Behavior – soft meowing in box, perks head up when box opened Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective T = NA P = NA R = NA BCS 3/5 (5/9) EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Grade 1/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NE Assessment: Mild dental dz, otherwise apparently healthy Medical Behavior Status – BLUE – allows all handling, tail twitching during exam, started to growl when placed on side for blood-draw, stopped and gave him a minute, got blood for CT from jugular and cat did very well for that Medical Status – 1 Asilomar Status – HEALTHY Fast Track? No Plan Already neutered. Okay for adoption pending behavior evaluation.
