Medical Summary DVM Intake Exam Estimated age: 4-6 Microchip noted on Intake? no History : stray Subjective: Flea allergy dermatitis/scabs around head / neck and ears Observed Behavior – shy, skittish, resents restraint, trys to flee Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 140 R = 20 BCS: 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: worn dentition, mild tartar, a few fractured teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: suspect female intact, no spay scar noted MSI: Ambulatory x 4, fleas, flea allergy dermatitis, mostly around head/neck/tail, no masses noted, poor haircoat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: flea allergy dermatitis, scabs, thin Plan: rabies fvrcp microchip flea tx pyrantel combo test convenia sq, ok for dose of steroids if very pruritic. Prognosis: fair to good SURGERY: Temporary waiver due to chronic dermatitis
