SAFE 9/28/18 CLIFFORD was adopted from the shelter last August after he was dumped with a housemate by former owner. NOW, Clifford has AGAIN been returned back to the shelter for reasons unknown, and needs a new furever home this time!! He has some ulcers in his nose and needs some follow up care for dental and possible weight loss. Please give this handsome boy a chance for a happily ever after this time – he is sad and nervous that he is back at the one place he hoped to never see again!

Subjective: Alert, hissing in carrier

Observed Behavior – Growls and hisses, but allows exam with towel as restraint

Evidence of Cruelty seen – None

Evidence of Trauma seen – None

Objective
BAR-H, MMs pink, BCS 7/9
EENT: Small ulcers on nasal planum – moist discharge. Eyes clear, ears clean, no ocular discharge noted.
Oral Exam: Moderate tartar and gingivitis
PLN: No enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Not distended, non painful, no masses palpated
U/G: Male neutered
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally

Assessment:
1. Ulcers to nasal planum – R/O trauma from carrier vs. indication of viral URI (ex: calicivirus)
2. Overweight
3. Dental disease

Prognosis: Good

Plan:
1. Monitor nasal ulceration
2. Recommend dental cleaning and gradual weight loss of 2-3 lbs
