Medical Summary scan: neg sex: male neutered (left ear tipped) est age: 5-7yrs S/O: Cat is Q/BAR, slightly dehydrated (inc. skin tent) Med Beh YELLOW allowed to be taken from shoreline, but once on table began hissing/growling and trying to flee Tried to scratch and fleed when sedated with 0.15cc Telazol IM EENT – dark debris AU; bilateral mucopurulent ocular and nasal discharge; blepharospasm and chemosis OU Grade 4/4 dental dz with ptyalism and halitosis Heart/lungs auscult wnl Abdomen wnl Coat dirty, especially paws CT – FIV+ A: URI, Severe dental dz, FIV+ P: Move to ISO and start on doxycycline and erythromycin (if able); recheck day 7 and 10; cleaned AU and applied Mometamax (will not be able to continue); cleaned eyes and nose and applied erythromycin OU PROGNOSIS: Fair
