SAFE 1/29/18 — 11 year old CLAUDE was purring too loud on his exam to check his heart thoroughly. He was dumped by his owner and needs a new home. CLAUDE has some diarrhea and a URI and some dental issues. CLAUDE is a sweet cat and would love to have a furever home this time.

Observed Behavior – Quiet

Objective

T = 101.7
P = 200
R = WNL
BCS = 3.5/9

EENT: Eyes clear, ears clean, dry green mucoid dis-s from nose, nasal congestion, no ocular discharge noted
Oral Exam: Missing 104-304 canines, peridontitis and calculus on molars and pre-molars
PLN: No enlargements noted
H/L: Purring too loud so could not ausculatate heart fully, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Fecal ball/matt – removed with scissors b/c painful, diarrhea on rear end
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Fecal ball on anus

Assessment
– URI
– Peridontal disease
– Diarrhea

Prognosis: Fair to good

Plan:
– doxycycline suspension 0.9 cc PO SID for 14 days
– cerenia 8mg PO SID for 3 days
– fortiflora in foot SID for 7 days

Monitor diarrhea – if continues consider metronidazole tx
Recommend dental work up
