Medical Summary DVM Intake Exam Estimated age: 2.5 months History : owner surrender Subjective: Observed Behavior – friendly Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 5/9 EENT: OU- mild clear mucoid discharge and conjunctival hyperemia, AU- large amount of dry brown debris, no nasal discharge seen but mild nasal congestion heard Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, purring but no murmur heard, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: FI MSI: Ambulatory x 4, no masses noted, flea dirt and dead flea seen CNS: mentation appropriate – no signs of neurologic abnormalities Assessment nasal congestion- R/O URI vs other conjunctivitis fleas- treated with Revolution 9/6/17 suspect ear mites- treated with Revolution 9/6/17 Plan continue doxycycline and erythromycin Prognosis: good SURGERY: Temporary waiver due to URI
