Medical Summary DVM Intake Exam Estimated age: Approx 9 weeks old Microchip noted on Intake? scan negative History : stray Subjective: Observed Behavior – friendly Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI, testicles not palpated, anal area open, enlarged and erythematous MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: diarrhea- R/O infectious vs dietary vs other abnormality at anal area- R/O irritation from diarrhea vs congenital abnormality vs fistula vs other Plan parvo-cite snap: negative start panacur 50 mg PO SID x 5 days start ponazuril 0.5 mls PO SID x 2 days SURGERY: Temporary waiver due to diarrhea
