DVM Intake Exam
Estimated age:4 weeks

History : stray
Subjective: not eating, but accepts food placed in mouth
Observed Behavior – BAR, tail raised
Evidence of Cruelty seen – n
Evidence of Trauma seen – n
Objective
P = 200 R = wnl BCS 3/9
EENT: purulent nasal and ocular d/c
Oral Exam: decid teeth
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated. thin.
U/G: female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: underweight, URI
Plan. doxycycline 50mg/kg/day, erythromycin ou bid , han feed
Prognosis: fair
SURGERY:
Temporary waiver due to age/uri
