Medical Summary 9/24/16 S/O: brought in by adopted, finished doxycyline for URI 9/17, has been eating/drinking, good energy level, but o has noticed some sneezing EENT: pink gums, mild gingivitis ,AU no discharge, but tip of AD – approx 1cm x1cm moderate erythema and hairless; also scab on base of AS ABN: SNP LN: NSF H/L: NSF M/S: amb x4, lean BCS 3/5 A: weight loss (was prev 9.0#, now 8.1#; r/o weight loss from being sick not eating) ear lesions: r/o trauma, infectious, other h/o URI- apparently resolved P: rec viralyz for 4 weeks (lysine supplement) send home with tresaderm AU on lesions both ears BID x14; if at 14days worse or not improved, rec re- eval 9/13/16 Returned home after treatment for URI; now has watery diarrhea. O: BARH. mm=pink, moist, CRT<2 s. EENT: No oculonasal discharge. ABD: Soft, non-painful. Palpation WNL. Integ: Normal skin turgor. A: Apparently healthy. Diarrhea most likely due to antibiotic therapy, stress, +/- abrupt diet change when returned home. P: Expect resolution over next 2-3 days, no treatment needed unless persists. Prognosis excellent.

Significant upper respiratory congestion. Quiet lung sounds. No resp distress noted. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male neutered A: URI, inappetent Short-term prognosis: Good P: Keep at ACC for a few days for SQF and nebulization 1. Doxycycline 0.9 ml PO SID x10 days 2. Nebulization BID x5 days 3. LRS 150 ml SQ SID x5 days 4. Monitor appetite. When eating, okay to go home. 1088
