[DVM Intake] DVM Intake Exam Estimated age: young adult (2-4yrs) Microchip noted on Intake? N History : stray Subjective: QAR, depressed mentation Observed Behavior – compliant, mentation not appropriate but able to perform complete exam Evidence of Cruelty seen – N Evidence of Trauma seen – Y- scabs on face and chin, dried blood AD Objective T = DNP P = WNL R = WNL BCS EENT: OS- significant blepharospasm, nictitans elevated, episcleral injection, mucopurulent discharge; OD- mild ocular discharge, mild blepharospasm; AD- dried blood visible edge of canal; AS- mild ceruminous debris Oral Exam: WNL- adult dentition present PLN: No enlargements noted H/L: Significantly dehydrated- skin tent lasts >10 seconds, NSR, NMA, CRT < 2,Lungs clear, eupnic ABD: Tense on abdominal palpation- abdomen feels doughy but no obvious masses or abnormalities, bladder not palpable U/G: Intact male- both testes present in scrotum; dark urine present on LH leg MSI: Reluctant to stand, skin free of parasites, no masses noted, significant swelling and scabbing of lips and chin/muzzle CNS: Depressed mentation, decreased to almost absent menace OU, palpebral present OU Rectal: DNP Assessment: significant dehydration, conjunctivitis vs. ulcer OS, scabbing/inflammation present on chin/muzzle, reluctant to stand w/ perceived abdominal pain on palpation; suspect possible trauma Prognosis: Fair to guarded Plan: IVC for fluid resuscitation, bloodwork, transfer to medical overnight for care SURGERY: Temporary waiver due to illness (dehydration, depressed mentation, abdominal pain, possible trauma)
Hx: suspected head trauma on presentation; dull mentation, traumatic conjunctivitis; started on IVF, cerenia and baytril overnight; blood work available for review CBC-mild neutrophilia, moderate lymphocytosis, severe monocytosis Chemistry-mildly elevated BUN (47), mild hyperphosphatemia, mildly elevated globulins, elevated tbili (3.8) S/O QAR, appears mentally appropriate; docile, friendly, allows all handling good appetite but appears painful severe facial swelling, traumatic conjunctivitis with some sl blood tinged residual mucoid discharge mild nasal abrasion with dried blood around nares mm pk, tacky; CRT <2 sec small abrasion with dried blood on ear AD eupnic, heart/lungs WNL soft, nonpainful abdomen is able to stand and walk, appears weak/slightly ataxic but ambulatory; no obvious orthopedic injuries A Head trauma-r/o HBC vs high rise vs other Leukocytosis Elevated tbili-suspect generalized inflammation P Continue IVF at 15 ml/hr Continue baytril IV SID Simbadol 0.55 ml SQ SID x 4 days Onsior 0.5 ml SQ SID x 3 days Terramycin OU BID x 10 days prognosis: fair to good
Hx: Suspected head trauma. Dull mentation and traumatic conjunctivitis on intake. Was started on IVF, Baytril, Simbadol, Onsior, terramycin eye ointment. Today pt is purring, head-butting and rolling around happily in his kennel. He is alert and aware of his surroundings. O: BAR-H, MMs pink and moist, BCS 5/9 EENT: Mild conjunctival erythema and periorbital swelling OU, multiple small scabs and small healing wounds on the face and inside the ears. No nasal or ocular discharge. H/L: NSR, NMA. Eupnic, quiet lung sounds. PLNs: Not enlarged Abd: Soft, nonpainful, no masses M/S/I: No swelling, wounds or lameness noted to body UG: Male intact A: Head trauma and traumatic conjunctivitis – healing well P: Discontinue IVF and Baytril. Continue Simbadol, Onsior, terramycin. 1088
Hx: presented obtunded with head trauma on 10/8; started on IVF, baytril, cerenia, onsior and simbadol; has improved significantly S/O BAR, soooo affectionate! good appetite mm pk, moist; CRT <2 sec; able to open jaw fully with no concerns mild healing abrasions along muzzle OU-mild blepharospasms eupnic, heart/lungs WNL soft, nonpainful abdomen with no palpable abnormalities ambulatory x 4 with no obvious lameness or ataxia A Head trauma-improving!! P okay to d/c baytril continue simbadol through tomorrow prognosis: good!
Notified oncall DVM of results of pets blood work. Placed 22g IVC in RF Cephalic vessel. Fluid started LRS at 15mL/hr, cerenia inj. 0.5mL IV, and Baytril 22.7mg/mL inj. IV SID per on call DVM.

ENRICHMENT NOTES: Asleep on bedding, facing front. Woke when door opened, lay in place, soft eyes and body. Allowed all pets, seemed a bit out of it. Cage Condition: No change Reaction to assessor: Chad immediately comes soliciting at the front of the cage. Reaction when softly spoken to: Chad remains soft in place. Reaction to cage door opening: Chad remains standing at the front of the cage, relaxed body posture, soliciting attention. Reaction to touch: Chad head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to being picked up: Allows the pickup and remains calm. ACTIVITY LEVEL: Moderate VOCAL: Somewhat chatty CHARACTER TYPE: Social Sweet Affectionate BEHAVIOR DETERMINATION: Beginner Behavior Asilomar H – Healthy BEHAVIOR SUMMARY: Chad interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.
