S/O: BARH. Active, attention seeking. Fresh food and clean litter available. No signs of reports of diarrhea
EENT: Mild conjunctivitis OU, no ocular or nasal discharge
HL: No sneezing, normal RR/RE
INTEG: Full coat
MS: Ambulatory x 4
UG: Male
Fecal: positive for roundworms
A: FeLV+, conjunctivitis, roundworms
P: Continue with current treatment and monitoring plan. Recommend another fecal after second dose of pyrantel in 2 weeks. Poor prognosis due to FeLV status

EENT: Eyes have mucoid d/c and mild entropion with conjunctivitis ou, ears have moderate crusty debris, no nasal discharge noted
Oral Exam: clean adult dentition, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: MI, 2 testicles descended
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities

Clean ears and applied gentizol
