DVM Intake Exam Estimated age: 4 weeks based on dentition and weight Microchip noted on Intake? Scanned negative History : Stray kitten found outside Subjective: Meows, allows all handling Objective BCS 4/9, MMs pink and moist EENT: Mild mucoid to serous discharge OU. AU moderate black granular discharge. Small amount of brown dry debris at corners of the nose. Oral Exam: All deciduous teeth are erupted PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Not distended, non painful, no masses palpated U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Conjunctivitis and URI 2. Ear mites Plan: 1. Cleaned ears and applied Tresaderm AU – continue cleaning q3 days and Tresaderm AU BID x10 days 2. Erythromycin OU BID x10 days 3. 06/22/17 13:50 Caydee A1116178 is a young female black & white DSH kitten who was found in a garbage bin outside in front of the finder’s significant other’s residence a little while ago in the Bronx. They aren’t able/willing to keep her but felt bad for Caydee and thought it best for her to be surrendered to us. The finder stated Caydee was friendly and allowed them to pick her up and place her in a box to be brought in to us at the BXAC. During intake Caydee allowed all handling. She would cling on to your hand or chest/shoulder if you rest her there for a minute. Caydee is very active and adventurous at such a young age; she’ll walk and or climb anywhere/thing she can. Caydee has the cutest meow/cry especially when you set her down. She seems to enjoy being held. Caydee allowed to be scanned (-), collared and taken pictures of even though it took several tries and was then placed into a kennel with ease.
