[DVM Intake] DVM Intake Exam Estimated age: 1-2 Microchip noted on Intake? no Microchip Number (If Applicable): NA History : Owner in hospital, 20 other cats surrendered Subjective: QAR, <5% dehydrated Observed Behavior – hunched in corner, very scared, tries to flee, when unable to flee, tries to bite Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 180 R = eup BCS 4/9 EENT: corneas clear but moderate conjunctivitis, elevated third eyelids, ears clean, green nasal dc, ocular discharge noted Oral Exam: limited PLN: No enlargements noted H/L: NSR, sinus arrhythmia, CRT < 2, referred upper airway sounds/congested ABD: Non painful, no masses palpated U/G: 2 desc testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; nails appear to be broken but unable to perform thorough exam due to temperament CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: NP Assessment 1 year old M DSH 1. Reaction to assessor:: Casey was lying down, calm and relaxed.
Reaction when softly spoken to:: Casey looks at the assessor with his ears tilted slightly to the side.
Reaction to cage door opening:: Casey becomes stiff and alert.
Reaction to touch:: Casey was hesitant of touch and attempts to shift away very slightly, but he allows gentle petting and remains in place while looking around at his surroundings.

Potential challenges comments:: Casey has displayed fearful behavior during their stay in the care center and appears uncomfortable with extended handling. Be sure to offer incentive such as treats or play time whenever the cat makes a small positive step.
