Lying on side nursing babies as I approach. She makes eye contact when spoken to and stretches her arms out. She lifts her head and opens her mouth when I open the kennel door – looks like she’s panting. Limited interaction at this time due to stress/possible medical condition. Sprayed feliway and added to socialization list.
MEDICAL:

History : Stray, brought in with two nursing kittens
Subjective:
Observed Behavior – BARH. Comes to front of cage but slowly, retreats back when reaches hand out, allowed all handling
Evidence of Cruelty seen – No
Evidence of Trauma seen – No

P = WNL R = WNL BCS 4/9
EENT: Corneal scarring OU, mild mucoid nasal discharge, ears wnl
Oral Exam: Mild dental tartar/gingivitis, pink mm
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic, sneezing
ABD: Non painful, no masses palpated
U/G: Female, lactating
MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: URI, conjunctivitis, lactating
Plan: Rec doxycycline 0.8ml PO q24 x 10 days and erythromycin OU q12 x 10 days
Prognosis: Excellent

Temporary waiver due to URI and lactating

EENT: Eyes clear, Conjunctivitis with purulent discharge OU, ears clean, no nasal discharge noted
Oral Exam: All deciduous teeth erupting, pink mm, no oral ulcers
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: Underage/underweight, URI, conjuctivitis
Plan: Rec doxycycline 0.15ml PO q24 x 10 days, erythromycin OU q12 x 10 days
Prognosis: Good
SURGERY:
Temporary waiver due to underage and URI
