Medical Summary DVM Intake Exam Estimated age: 5 w Microchip noted on Intake? History : O surrender – reports red eys and mucousy stool. Ate okay. Subjective: BAR, no c/s, slight ocular d/c – dried. Crusty tail – dirty. Slightly thin and unkempt. Observed Behavior – Friendly Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = 102. 6 P = 200 R = eup BCS= 3/9 EENT: Eyes – sclera injected with focal areas of hemmorhage, slight dired mucoid d/c, cornea clr, no blepharospasm, no chemosis. Ears cln. No nasal discharge noted. MM pk. Oral Exam: normal deciduous teeth, NSF. PLN: No enlargements noted H/L: NSR, NMA. MM pk, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated. Pot bellied, slightly fluid filled. U/G: female. MSI: Ambulatory x 4, skin free of parasites, no masses noted, slightly thin, BCS= 3/9. Unkempt haircoat. Dried debris/ fecal matter on tail and under belly. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done. Leaking light brown stool but no blood. Assessment: 5 wk female. Diarrhea Scleral injection but does not appear to be full conjunctivitis. Plan: FVRCP and other usual intake or 5 wk. Parvo test – negative. Monitor eyes. No tx for now. Cleaned tail and belly. Prognosis: Excellent. SURGERY: Temporary waiver due to underage.
