Hello, my name is Bubbles. My animal id is #17384. I am a desexed male brown tabby cat at the Brooklyn Animal Care Center. The shelter thinks I am about 15 years old. – P
I came into the shelter as a owner surrender on 31-Dec-2017, with the surrender reason stated as person circumstance- cannot afford to care for.
Bubbles is at risk for medical concerns. Bubbles has a history of gastrointestinal issues and is geriatric with possible other underlying issues. Bubbles has been friendly in the care center and allows all handling.

DVM Intake Exam Estimated age: 15yrs Microchip noted on Intake? no History: O/S due to medical issues Subjective: Cat is BAR Observed Behavior – continuously growled during examination and treatments, but did not escalate to aggression Evidence of Cruelty seen -no Evidence of Trauma seen -no Objective T = NA P = 170 R = wnl BCS 1-2/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Grade 3/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Geriatric underlying – cachexia, hx of chronic vomiting, and inappropriate urination Prognosis: Poor Plan: Pull blood for CBC/CS/T4. Waiting on medical records. Moving to room 8 with medical sign since unable to keep in KSC due to quarantine. Monitor appetite, vomiting, urine and BM. Further Tx pending BW results. SURGERY: Already neutered

Progress exam History: O/S 12/31 due to medical issues. Was previously being treated at Lefferts Animal Hospital for chronic GI issues. Waiting on medical records to be sent over. CBC: mild non-regenerative anemia 28.8 (30.3 – 52.3 %), mild leukocytosis 18.8 (2.87 – 17.02 K/µL) with neutrophilia 11.33 (1.48 – 10.29 K/µL) with suspect bands, mild monocytosis 0.91 (0.05 – 0.67 K/µL) Chem: wnl T4: wnl Subjective: BAR, ~5% dehydrated. Eating canned food. Very friendly and purring. Underweight. No c/s/v/d noted in cage. Objective P = wnl R = wnl BCS 1-2/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: adult dentition, moderate dental disease PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MN MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt, underweight CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: Suspect geriatric underlying – cachexia, hx of chronic vomiting, and inappropriate urination Dental disease Prognosis: Fair Plan: Waiting on medical records. Start LRS 20ml/kg SQ SID x3d until 1/4 Start cerenia 1mg/kg SQ SID x3d until 1/4 Monitor appetite, vomiting, urine and BM.

Monitor for vomiting/diarrhea, dehydration – owner reports chronic GI issues S/O: QARH. Allows handling. Eating well. Normal stool in litter box. No diarrhea or vomiting noted. EENT: Nuclear sclerosis, no ocular or nasal discharge HL: Normal thoracic auscultation, no sneezing INTEG: Unkempt hair coat MS: Ambulatory x 4, underweight A: Geriatric Hx of chronic GI issues – no GI issues noted/reported while in shelter Mild non regenerative anemia Mild leukocytosis with neutrophilia, suspect bands; chem and T4 WNL Records/blood work from referring vet show history of chronic pancreatitis, Spondylosis–T12-13, and in all intervertbral interfaces of lumbar region. P: Continue with current treatment and monitoring plan. Poor long term prognosis

KNOWN HISTORY:: Bubbles was brought in without information on his behavior history or tendencies in a home environment.
MEDICAL BEHAVIOR:: Very friendly and purring.
ENRICHMENT NOTES:: 01/02/18 Standing in front of kennel, body relaxed and ears forward. Makes eye contact. Remains calm as cage door opens and allows all petting. Doing well!
Cage Condition:: Cage is neat
Reaction to assessor:: Bubbles seems curious and slowly comes forward to interact with the assessor.
Reaction when softly spoken to:: Bubbles sniffs through the bars then starts grooming himself.
Reaction to cage door opening:: Bubbles remains calm.
Reaction to touch:: Bubbles sniffs the assessor’s hand and perks up when pet along his back. He leans in for rubs on his cheeks, his tail rises and he gives head butts.
Reaction to being picked up:: Bubbles tenses up, vocalizes and tries to wiggle out of the assessor’s arms.
