Medical Summary Sex: Male intact Scan: Negative Estimated age: 2 years based on dentition and size Fleas: No flea dirt. S: Friendly, allows all handling, easy to work with O: BAR-H, BCS 5/9, MMs pink and moist EENT: 0.5×1.5 cm rectangular superficial wound on left upper lip – erythematous, alopecic, no discharge or swelling. No discharge OU, AU, nose. Clean teeth but left maxillary canine is fractured. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male intact, testicles soft and symmetrical. A: 1. Fractured left max canine tooth 2. Superficial wound on left upper lip – likely traumatic in origin, possibly ringworm Short-term prognosis: Excellent P: 1. Gave 1 ml pyrantel PO and Activyl 9+ during exam 2. Wood’s lamp face lesion once pt is transported to MACC 3. Recommend extraction of fractured tooth to prevent tooth root infection 1088
