Medical Summary DVM Intake Exam Estimated age: approx. 10 mos Microchip noted on Intake? no History : stray cat Subjective: BAR on presentation Observed Behavior – Allowed all handling, attention seeking Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = Not performed P = 180bpm R = eupneic BCS = 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Clean adult dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, Severely distended abdomen with hard masses present throughout the abdomen U/G: Non-lactating, Intact female MSI: Ambulatory x 4, skin free of parasites, no masses noted. On dorsal aspect of right metatarsal region, there is a small, round area of alopecia with mild flaking, measuring approx. 0.5 cm in diameter. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Approc 10 mo. old FI DSH with: Late gestation Round area of alopecia – r/o focal ringworm vs. superficial pyoderma vs. scar vs. previous injury vs. other. Plan: Wood’s Lamp – positive DTM plate taken – To be checked daily x 14 days Bathed with Lime Sulfur Dip – continue 2x/week until negative culture Prognosis: Good for recovery SURGERY: Temporary waiver due to ringworm
