Subjective: very sweet kitten, BAR Observed Behavior – eating solid food well on his own, ambulatory Objective T = P = 180 R = 28 BCS 5/9 EENT: Watery eyes with slight discharge, nictitating membranes pronounced; sneezing with mild serous nasal discharge Oral Exam: Pink mucous membranes, no ulcerations seen PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment conjunctivitis underage/underweight Plan doxycycline 0.1 ml PO SID x 10 days erythromycin OU BID x 7 days monitor app, force feed if not eating well on his own Prognosis: good SURGERY: temp waiver due to underage/underweight
