Medical Summary History Found under a pallet at a hardware store, no hx given Subjective Laterally recumbent, paddling his legs, allows handling Objective T = 100.4 BCS 2/9, MMs pale pink EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Mild dental disease PLN: No enlargements noted H/L: Grade 1-2/6 systolic heart murmur. NSR. Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male neutered MSI: Laterally recumbent, no fractures or injuries noted. Large amount of flea dirt. CNS: Looks around and makes eye contact. Paddles legs when placed on left side – calmer when placed on right side. Front legs very stiff. Blood glucose: 115 Assessment 1. Emaciated 2. Heart murmur 3. Neurological signs – appears to be vestibular R/O head trauma vs. infection (rabies cannot be ruled out although no bite wounds are seen) vs. other cause 4. Flea dirt Plan: 1. Set up in Medical with IVC – LRS at 5 ml/hr 2. Syringe feed if necessary 3. Consider EHR if condition declines 1088
