O: BAR, much more social today and seeking attention at front of cage. mm=lt pink, moist, CRT<2 s.
EENT: Ointment present OU, no discharge. Mod. mucoid nasal discharge and mild crusting, with congestion.
H/L: Purring. Clear lung sounds.
ABD: Palpation WNL.
INTEG: Crusting on face below nasal area with small sores. Mild decreased skin turgor.
A: Clinically improving.
P: Continue current therapy and monitoring. Fair to good prognosis.

Recheck lethargic kitten with URI
O: QAR. mm=pale pink, tacky, CRT<2 s. No interest in food.
EENT: Severe congestion, mod. mucopurulent nasal discharge.
Chemosis and serous discharge OU, worse OD.
H/L: Lungs clear, no murmurs.
ABD: Palpation WNL.
INTEG: Decreased skin turgor.
A: Severe URI with dehydration and inappetance, stable but serious condition.
P: Continue current therapy with Doxy, LRS, nebulization, and hand feeding.
Guarded prognosis.
Continue to monitor.

radiographs: lateral whole body — no signs of fluid in chest or abdomen, lungs wnl; large amount of gas in stomach and intestines. no other significant finding
a: severe uri, emaciated, dehydrated
p: administered lrs 30ml sq, doxycycline 0.3ml po and nutrical — rec ehr if condition declines
will monitor in medical fair/poor prognosis
rec doxycycline 0.3ml po q24 x 10 days, lrs 30ml sq q24 x 3 days, nebulization q24 x 3 days

Small, social, kitten, lethargic, appears to be very sick. Limited interaction because of condition.
