Medical Summary DVM Intake Exam Estimated age: 3 mo based on dentition Microchip noted on Intake? neg on inake History : stray Subjective: BARH, underweight, very friendly, allows handling Objective: BCS: 3/9 EENT: Eyes clear with mild conjunctivitis OD, ears clean, bilateral serous nasal discharge Oral Exam: all kitten dentition present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, only one testicle descended MSI: Ambulatory x 4, skin free of parasites, no masses noted, scab around right eye (lateral to right eye) CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: grossly normal Assessment: 3 mo MI DSH 1, underweight 2. mild URI Plan 1. doxycycline 0.15ml PO SID x 10days 2. erythromycin OD BID x 10 days 3. recheck in 5 days Prognosis: good with care SURGERY: Temporary waiver due to underage
