Medical Summary DVM Intake Exam Estimated age: approx 6 months Microchip noted on Intake? neg History : owner surrendered several cats due to overpopulation in the house Subjective: QAR Observed Behavior – nervous, somewhat unsocialized; non aggressive but difficult to handle; good appetite Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 5/9 EENT: Eyes clear, crusting nasal discharge but no active discharge or sneezing noted; AU-dark discharge, pruritic Oral Exam: mm pk, moist; mild hypersalivation; deciduous canines present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Ear mites Plan gave 0.06 ml ivermectin SQ, repeat dose in 2 weeks Prognosis: excellent SURGERY: Okay for surgery
