S/O: Cat is Q/BARH BCS 5-6/9, some muscle atrophy of epaxials and hind legs Approx 1cm firm mass left lower lip just caudal to canine Grade 3/4 dental dz Heart/lungs auscult wnl Abdomen tense, but unremarkable Coat wnl Changed Med Beh to BLUE cat was hissing before we got him out of kennel, was tense for exam, and growled during abdominal palpation A: Geriatric underlying – firm mass left lower lip P: Recommend rescue placement; mass should be removed and submitted for histopathology to determine malignancy PROGNOSIS: Fair-Poor

04/18/16 17:06 Basic Information: Bear is a 12 year old male domestic medium hair cat who was surrendered due to his previous owner falling into finacial hardship. Socialization: Around starngers Bear is shy for a few minutes and will play gently with adults. Bear has never been around any children, dogs or other cats so it is unknown how she will react to being around them. Bear has never bitten another person or animal. Behavior: Bear tends to have litter box accidents and will sleep when he rides in a car. Bear’s previous owner did not attempt to give Bear a bath or trim his nails so it is unknown how she will react to these events. Bear enjoys being brushed, picked up and held, placed in a carrier, and disturbed while he is resting. For a new Family to Know: Bear is described as: friendly, affectionate, mellow, shy, independent and quiet. with a medium activity level. When at home Bear will sit on his owner and likes to stay in the same room as his owner. Bear likes to play with chasing toys and has been kept indoors only. Bear likes to sleep on the furniture and on his owners bed. Bear was fed wet food from the brand fancy feast twice a day. Bear is litter box trained but often defecates outside of the litter box. Bear has used an uncovered litter box with pine scented pellets for litter. Bear used a circle shaped scratching post made of carpet. Intake Behavior: Bear allowed staff to collar, scan for a microchip and take pictures with no signs of aggression.

Reaction to assessor: Bear was already watching the assessor from the front of the cage When spoken quietly: Bear stays at the front of cage watching the assessor calmly Reaction to door cage opening: Bears remains soft and relaxed Reaction to touch: Bear solicits petting and attention Try picking cat up: Bear is tolerant of this type of handling Behavior Determination: Beginner At the time of the assessment Bear was displaying friendly behavior, comes to the front of the kennel, interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. Bear is displaying behavior appropriate for a beginner cat owner.
