Medical Summary DVM Intake Exam Estimated age: ~2months Microchip noted on Intake? no History : Stray Subjective: Observed Behavior – QAR. ~5% dehydration. Offered A/D and ate well with no coaxing. Allowed all handling. Vocal with blood draw Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = WNL R = WNL BCS 3/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Pale pink mm, teeth erupting normally PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, wet fecal staining around hind end CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Underweight, diarrhea, lethargic — parvo test neg Plan: LRS 30ml given SQ by LVT. Ponazuril and pyrantel given. Rec metronidazole 0.1ml PO q12 x 7 days (first dose given) Prognosis: Good SURGERY: Temporary waiver due to underweight and diarrhea
