SAFE 5/22/18 BAM BAM was brought to the shelter because of litterbox issues. He is a bit hissy and upset and will need a home asap.

Intake Date: 5/18/18

Intake Type: Stray

Medical Behavior: Blue

Sex: Male

Age: 4 years

Original Location: 11217

[LVT Intake Exam]

Microchip Scan: negative, could not place
Evidence of Cruelty: no
Observed Behavior: allowed handling but then began to hiss and retreat to carrier, escalates quickly and will spit, left alone to calm
Sex: intact male
Estimated Age: reported 1y but seems closer to 3-4y

Subjective: reported inappropriate urination in home (possibly behavioral, see memos)
Eyes: clear
Ears: clean
Oral Exam: mild staining, odorous, unsure of hyper salivation from ddz or fear
Heart: WNL
Lungs: WNL
Abdomen: WNL
Musculoskeletal: WNL BCS 5.5/9
Mentation: BARH

Preliminary Assessment: seemingly healthy

Plan: DVM intake, CT and MC when calm or at Sx
