Ate well. Does not appear painful.

DVM Intake Exam
Estimated age: ~10 months
Microchip noted on Intake? positive
History : stray with bilateral forelimb injury
Subjective: BARH, sneezing and mildly congested
Observed Behavior – very sweet but seems painful in carpi. Easily handleable. Did well for all medical handling and procedures
Evidence of Cruelty seen – no
Evidence of Trauma seen – yes
Objective
P = wnl
R = eupneic
BCS 5/9
EENT: Eyes have mild mucoid d/c ou, ears clean, mild serous nasal discharge noted
Oral Exam: adult dentition with mild dental staining, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: MN
MSI: Ambulatory x 2, forelimbs are lax/bent at mid-carpus and seem painful on palpation, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment:
Complete closed transverse fractures of the distal radius and ulnar diaphyses bilaterally
URI
Mild pneumothorax
Mild pulmonary contusions
Plan: Continue to monitor while at BACC
Sedated with telazol 0.1ml IM
2 view RFL radiographs-complete closed transverse fracture of the distal radius and ulnar diaphyses bilaterally
2 view CXR-mild pneumothorax with mild pulmonary contusions
Start doxycycline 10mg/kg PO SID x10d until 11/25
Start erythromycin OU BID x10d until 11/26
Move to iso
Start simbadol 0.24mg/kg SQ SID until 11/18
Start onsior 6mg PO SID x3d until 11/17
Placed bilateral spoon splints-stirrups, cotton padding, spoon splint, cotton padding, vet cling, vetwrap
Rec surgical repair of limbs bilaterally for best outcome, will need splint change every 2 weeks until able to repair
Prognosis: Good with surgical repair
SURGERY: neutered
