Medical Summary 1/27/17 Sex: Female, reported spayed Scan: Negative Estimated age: 3 years as reported by previous owner Fleas: No flea dirt. S: Calm, allows all handling O: BAR-H, BCS 5/9, MMs pink and moist EENT: AD: mild dry brown granular discharge. AS: Moist slightly purulent discharge with dry sandy discharge surrounding this. Severe dental disease – tartar, halitosis, “supereruption” of canines, gingivitis, teeth chattering with administration of oral dewormer. Clean eyes and nose. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Female, reported spayed A: 1. Ear mites AU, otitis externa AS 2. Severe dental disease Short-term prognosis: Good-fair P: 1. Administered pyrantel 0.5 ml PO during exam 2. Cleaned ears, administered Otibiotic in left ear – continue otibiotic in this ear BID x5 days, clean both ears in 3 days and 6 days and 9 days 3. Administered ivermectin 0.15 ml SQ during exam – repeat in 2 weeks to definitively clear ear mites 4. Recommend dental cleaning with rads and likely extractions as soon as pt is placed! 1088
