Medical Summary 05/23/17 DVM Intake Exam Estimated age: 1 yr Microchip noted on Intake? No. History : Stray cat, no medical concerns noted. Subjective: Observed Behavior – Social, very curious about her surroundings. Became a little tense with handling but allowed all exam and treatments. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=200, RR=24. BCS=5/9. ORAL: All adult teeth present, no oral lesions. EENT: Eyes clear, ears clean, no nasal discharge noted. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD/UG: Non painful, mildly distended abdomen, no masses/organomegaly or feti palpated. Female, prominent nipples, no OVH scar found. MSI: Ambulatory x 4, no lam,eness. Skin free of parasites, no masses noted, healthy hair coat. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Apparenyl healthy young adult cat, suspect early pregnancy. P: OK for OVH. Excellent prognosis.
