EENT: Eyes – bilateral crusts, ears clean, no nasal discharge noted
Oral Exam:
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G:
MSI: Ambulatory x 4, skin free of parasites, no masses noted, alopecia and crusting ventral thorax, but no obvious erythema – suspect secondary to purulent discharge coming from wound on RF paw; cannot say as to cause of wound, possibly caught paw in fence; other kitten has no obvious injuries.
CNS: mentation appropriate – no signs of neurologic abnormalities

Medical Summary DVM Intake Exam Estimated age: 6-7 weeks Microchip noted on Intake? no History : Two kittens found as strays in yard. Subjective: Kittens are Q/BARH. Observed Behavior – Stays towards back of carrier, but does not hiss when approached. Allows most handling with minimal restraint, does attempt to bite at gauze when eyes and nose wiped. Will try to flee. Allows wounded paw to be touched without signs of discomfort, but shakes when held. Evidence of Cruelty seen – no Evidence of Trauma seen – yes Objective T = NA P = 200 R = wnl BCS 4-5/9 EENT: Eyes – bilateral crusts, ears clean, no nasal discharge noted Oral Exam: healthy deciduous dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl MSI: Ambulatory x 4; still weight bearing on RF despite wound, live fleas, no masses noted, alopecia and crusting ventral thorax, but no obvious erythema – suspect secondary to purulent discharge coming from wound on RF paw; cannot say as to cause of wound, possibly caught paw in fence; other kitten has no obvious injuries. Wound consists of multiple ulcerated scabbed areas with purulent discharge, after cleaning there appears to be healthy granulation tissue under the scabs. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Underage/Underweight, Conjunctivitis, Wound (RF paw) Plan: Soaked paw in chlorhexidine. Gave 0.13cc Clavamox PO – continue BID x 10d. Cleaned eyes and applied erythromycin OU – continue BID x 10d. Keep in medical to monitor wound. Soak and/or clean with chlorhexidine daily for 3-5 days. Prognosis: Good SURGERY: Temporary waiver due to Underage/Underweight
