Medical Summary DVM Intake Exam Estimated age: 3-4 YO based on size and teeth Microchip noted on Intake? neg on LVT intake History : surrendered to shelter (hissing behavior seen by friend of owner) Subjective: okay initially, but becomes agitated with time/hissing and growling with progressive examiation. limited PE d/t behavior Objective BCS: 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: very limited. no oral masses seen when patient hissing PLN: Not examined H/L: NMA, CRT < 2, Lungs clear, eupnic ABD: tense on palpation U/G: male intact, both testicles descended MSI: Ambulatory x 4, skin free of parasites today (fleas seen on LVT intake), healthy hair coat FACE skin: Minor excoraited skin on face (over top right aspect of nose) CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: grossly normal Assessment: 3-4 YO MI DSH 1. Apparently healthy Plan no treatment indicated (just neuter) Prognosis: good SURGERY: Okay for surgery
