Microchip: Scanned negative Sex: Female; unable to shave due to temperament Age appears young but teeth indicate older cat; confirm with DVM intake Mentation: appropriate Eyes: clear Ears: clear Nose: clear Teeth: upper right canine chipped; teeth are dull, stained, mild tarter. If abnormal BCS: very slightly underweight but still appears healthy; good appetite Skin: dry; no flea dirt or parasites found Hair Coat: thinning of hair in areas appropriate for FAD (ears, lower spine); scanned negative under Wood’s Lamp. Declawed: no Any injuries: no Behavior: crouched down, eyes wide, back of kennel. Stiffens on approach but if move slowly will allow lifting. Very tense during exam; fearful of clipper; did not shave for spay scar. Medication: none
