ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Calm, Sweet KNOWN HISTORY: Amalfi was brought in as a stray, so we don’t have any behavioral history or tendencies in a home environment. MEDICAL BEHAVIOR: 8/28/17- Subjective: When being removed from kennel, pt hissed and growled. Quiet, sits on table, allows handling but gets tense and vocalizes when handled too much. EVALUATION: Cage Condition: No change Reaction to assessor: Amalfi remains neutral, lying down on his cage bedding during the approach. Reaction when softly spoken to: Amalfi remains immobile in place. Reaction to cage door opening: Amalfi remains alert with soft eyes, ears erect and forward. Reaction to touch: Amalfi slowly head bunts the assessor’s hand, but seems unsure and began to lip licking. Reaction to being picked up: She becomes antsy then jumps back into the cage. BEHAVIOR SUMMARY: Average Amalfi interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

Objective BAR-H, MMs pink and moist, BCS 9/9 EENT: Moderate black granular discharge AU. Eyes clear, no nasal discharge noted. Oral Exam: Severe tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Large, non painful, no masses palpated U/G: Female, spay scar visible on ventrum MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment 1. Obese 2. Dental disease 3. Likely ear mites – was given ivermectin on LVT intake Plan 1. Recommend weight loss of 4-8 lbs 2. Cleaned ears during today’s exam; clean again in 5 days and repeat ivermectin in 10-14 days 3. Recommend dental cleaning in the near future Prognosis: Good 1088
