Initial exam (3/19): DVM Intake Exam Estimated age: estimated 8 year based on dentition History : stray found with swollen foot. Subjective: laying in carrier, rubbing face and attention seeking Observed Behavior – trouble walking, easily handlable Evidence of Trauma seen – yes- wound to right front carpus Objective 8% dehydrated P = 180 R = wnl BCS 3/9 EENT: pale mucous membranes; Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: calculus and tartar on molars, missing several incisors PLN: No enlargements noted H/L: tachy mm; NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: mass felt in the cranial abdomen, all intestines thickened; Non painful U/G: neutered male MSI: right front leg is extremely swollen to the eblow; toes extremely painful, 3 cm open wound on lateral capus with pus. Ambulatory x 3, unkempt hair coat, no masses noted CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: clean anus on external exam Assessment; underweight, mass in cranial abomen (open); swollen right front leg with wound on carpus (abcess v. ulcerated mass v. other) Prognosis: fair
Second exam (3/19): Blood-work interpretation cbc- Moderate macrocytic regenerative anemia with neutrophilia chem- muscle wasted with normal creatnine and elevated BUN (108 mg/dL)- dehydration v. kideny disease (bladder soft non-painful) elevate calcium (15.3mg/dL) with normal albumin- neoplasia conclusion- neoplastic elevated calcium strongly suspected right front limb rads- soft tissue swelling around carpus and 1st digit. suspicious of osteomyeliti on the firs digit whole body rads- throrax- lateral is suspicious of a hemo/pyo/othe thorax, soft tisue opacity between longs and sternum. heart elevated from sternum and lungs are consolidated. abdomen- gas filled colon. small gas bubbles possibly in small intestines. kidneys bilaterally symmetrical
Third exam (3/20): Hx: RFL abscess and pathologic fractures – concern for abscess extending into bone vs. neoplasia causing secondary abscess. Mass palpated in abdomen. S: Alert, purring, leans into head pets O: BAR, estimated 5-8% dehydrated based on decreased skin turgor, BCS 2/9, MMs pale and tacky EENT: No discharge OU, AU, nose. Severe dental disease. PLNs: Not significantly enlarged. H/L: Eupnic, not ausculted. Abd: Soft, no pain on palpation, no masses palpated. Large bladder, abdominal mass palpable today M/S/I: Distal RFL swollen with purulent discharge. No other lesions noted. Neuro: Alert and appropriate, no sign neurological deficiencies A: 1. Emaciated, dehydration 2. RFL abscess – concern for abscess extending into the bone vs. neoplasia causing an abscess Short-term prognosis: Fair-poor
