**SAFE 09/12/16** SECOND CHANCE FOR THIS GREAT GUY WHO NEEDS HELP!! MAKE IT COUNT!! ZELDA NEEDS SOME MEDICAL COMFORT FOR HIS MOUTH! Looking at Zelda’s photo, you can almost see the pain in his eyes. Zelda’s need appropriate and immediate treatment for his infected mouth but doesn’t seem to be getting anything but a death sentence at ACC. This neutered seven year old fella earned an awesome AVERAGE behavior rating and deserves to saved. Step up for this great boy tonight!

I came in the shelter as a STRAY on 09/06/2016 from NY 11426, owner surrender reason stated was STRAY.

9/11/16: S: recheck exam; ate well this morning O: QAR, hissing when handled severe dental disease pale mm’s, mm’s moist, decreased skin turgor no murmur, regular rhythm lungs clear and eupneic abdomen- snp BCS 2/9; shaved caudodorsum A: anemia emaciation severe dental disease elevated BUN (75) on 9/8/16 P: entered request for repeat CBC/chem today if no improvement in bloodwork or getting worse, recommend euthanasia continue current meds, SQ fluids 9/9/16 S/O-QAR, eating well, hydrated, IVC in place, decreased oral malodor, but dental disease or impacted food in molar region present, SNP on abdominal palpation, bloody material (stool vs urine vs vomit) present in litter box P-continue with current treatment plan of IVF, cerenia, clavamox d/c SQ fluids 9/8/16 S/O-QAR, tries to flee but allows handling, +/- hydrated, no interest in food this AM EENT-mild d/c AU, mucoid d/c OS, COD, no nasal d/c H/L-no m/a, clear and eup GI-severe dental disease with visible non-viable teeth, SNP, NMP MSI-BCS 2/5, amb x 4, shaved caudal dorsum, otherwise ok haircoat A-bloodwork changes (18% anemia, 25 K/ul leukocytosis, 22K/ul neutrophilia, hyperproteinemia, elevated BUN 75 mg/dl, with electrolyte changes-164 NA, 129 Cl) P-placed IVC for IVF (9% NaCl at 9.5 ml/hr-1.5x maintenance) Cerenia 10 mg/ml-0.25 ml SQ SID x 5 days mirtazipine 15 mg-1/4 tab PO every 72 hours in addition to the severe dental disease, renal disease may be contributing to the ptyalism and how patient is feeling. recommend placement to have IVF, monitoring for electrolytes and renal values if no improvement and continues to have no interest in food today despite IVF and added meds, recommend EHR

Reaction to touch: Zelda accepts the stroke and appreciates petting on the head and body. Reaction to Being Picked up: She becomes antsy then jumps back into the cage. Placement determination: Average Zelda interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. Part of TNR program Neutered male–Left ear tipped Scan negative for a microchip About 7 years old–hard to estimate age based on the condition of the teeth OD=minor discharge and squinting OS= clear and WNL Nose= No discharge Ears are waxy Teeth= Poss. severe case of DDZ vs other–On a vet check Severe mouth infection. While drooling blood was seen and has the infection odor breath Emaciated Coat was matted. Matts were shaved off Cobo test result= Negative As of now no V/S/C/D As of now no signs of URI NOSF Will keep in medical for observation Was nervous and did try to flee a couple of times. Overall, he did allow handling
