Medical Summary DVM Intake Exam Estimated age: 9mos Microchip noted on Intake? no History : Subjective: very friendly, head butting and purring Observed Behavior – docile, allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = purring, unable to obtain HR R = purring, but eupneic BCS 4.5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild gingivitis, pink gums PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, both testicles descended normal consistency MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment apparently healthy MI Plan ok for neuter and adoptions Prognosis: good SURGERY: Okay for surgery
