I am a neutered male black domestic sh mix. The shelter thinks I am about 4 YEARS old.
I came in the shelter as a STRAY on 02/18/2017 from NY 11417, owner surrender reason stated was STRAY.

03/02/2017 Exam Type CAGE EXAM – Medical Rating is 1 – NORMAL , Behavior Rating is EXPERIENCE, Weight 9.9 LBS.
03/02/17 Finished terramycin, recheck conjunctivitis OS S/O: QAR. Eating well. EENT: Eyes clear, small amount of ointment in eyes, no conjunctivitis noted, no nasal discharge HL: No sneezing A: Conjunctivitis resolved, treated for ear mites on 2/19 P: Continue to monitor while at BACC. Excellent prognosis Scan negative S: BAR on presentation, patient had limited exam due to bite history and was netted initially for exam. Tolerable of minimal handling and able to be restrained with a towel. EENT = mildly hyperemic conjunctiva with mild chemosis OD. Mild corneal cloudiness with a small (approx. 3 mm) red iris pigmentation that appears to be an extension of a previous corneal lesion/ulcer, OD. Clear OS. Severe black debris AU with a large (approx. 0.5-1 cm) scabbed wound on the caudal aspect of the right pinna. Teeth have mild gingivitis, with mild gingival recession. No nasal discharge H/L = nma, nsr, eupneic, clear lung fields Abdomen = snp, nmp GU = intact male LN = wnl CNS = A+A Integ = clean coat (visually) MS = BCS: 5/9, Amb x 4 A: Approx. 3-5 yr. old intact male DSH with Conjunctivitis OD Otitis Externa AU – r/o secondary to ear mites vs. other Pinnal scabbing – secondary to OE Mild dental disease Px: Good P: Start Terramycin OD BID x 10 days (if possible) – patient was tolerable for administration during exam Apply revolution to treat for Ear mites Clean ears while under anesthesia for surgery

02/26/2017 BEHAVIOR EVALUATION – EXPERIENCE

Zack was brought to the shelter as a stray, so his behavior in a previous home is unknown. ENRICHMENT NOTES: 02/19/17 Appears to be faking sleep, seems really uncomfortable, holding body tightly, almost seems sick or injured. Dull, depressed, quiet, unmoving. Allows petting all over but flinches a bit with the touching. Appears to have eaten a bowl of wet food overnight, crunchy untouched. Sprayed Feliway, gave a bowl of fresh wet and some treats. Initial med exam pending. 02/26/17 Very quiet, curled up at front, back against kennel door. Doesn’t react much when I touch. Lifts up and turns around when I open cage door, head and eyes are held low, as usual. Not a medical opinion, but via interaction it seems very much that Zack has trouble seeing and/or sensing things around him. Allows touching/petting all over but flinches a little each time, almost as if he’s not aware of the approaching hand. Seems to struggle a little to find a bowl of fresh wet food I give him, seems to find it by smell. Continues to eat wet food well, doesn’t seem to have much interest in dry food. Allowed me to pick him up, but was uncomfortable. Remains quiet throughout, with head down. Has never shown any aggression, has preferred to be on his own, resting. Finder reported that Zack seemed ill/injured when he found him. A sweet cat that seems like he’ll need some XTLC. ACTIVITY LEVEL: Mellow VOCAL: Quiet, CHARACTER TYPE: Shy, Mellow, Independent BEHAVIOR SUMMARY: Please note that Zack is being treated for conjunctivitis at the time of evaluation. It is difficult to determine at this time how the medical condition may be affecting the behavior. Please note that his behavior may change once his medical condition is addressed and he has recovered. Behavior observations _X_ Experience Zack tolerates attention and petting but may be fearful or stressed in the shelter. He may be a little more independent, and may need time to warm up to his or her new home. We recommend that this cat go to a home with experienced cat parents.
