Hello, my name is Wizard. My animal id is #12227. I am a desexed female black cat at the Manhattan Animal Care Center. The shelter thinks I am about 4 years old. – P
I came into the shelter as a aco impound on 10-Nov-2017.
Wizard is at risk for medical reasons. She has contracted an Upper Respiratory Infection, which is contagious to cats. The condition is treatable in home with oral medication. Wizard shows no behavioral concerns.

[DVM Intake] DVM Intake Exam Estimated age: approx. 4yo Microchip noted on Intake? 98511200255659 History : abandoned stray Subjective: BAR, friendly, good for medical handling, purring, eating during PE Observed Behavior – Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective T = P = wnl R = eup BCS 7/9 EENT: Ocular changes OS, possibly phthisical, it appears that the TEL is covering most of the eye, unable to visualize full cornea. Ears clean, no nasal or ocular discharge noted. Concern for URI signs during LVT intake, but none noted during PE Oral Exam: mild dental calculus PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FS, spay scar MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: normal externally Assessment Ocular changes OS – r/o congenital vs trauma vs due to past infection otherwise apparently healthy Prognosis: good Plan: available for transfer/adoption ctm OS, consider full eye diagnostics once placed monitor for any URI signs
OD: Moderate serous ocular discharge OS: Severe serous ocular discharge, elevated third eyelid, appears to have moderate blepharospasm although enophthalmia may be the case instead, or possibly corneal ulcer – difficult to visualize cornea due to the elevated third eyelid. Normal response to globe retropulsion. One sneeze noted during exam. Pt definitely has conjunctivitis and URI; may also have pre-existing changes to OS secondary to genetics or trauma. Move to isolation, start doxycycline 1.1 ml PO SID x10 days, erythromycin OU BID x10 days. Recheck exams every few days. 1088

KNOWN HISTORY:: Wizard was brought in as a stray, so there is no information on their behavior history or tendencies in a home environment. She appeared fearful during intake, but allowed handling.

Reaction to assessor:: Wizard was calm, relaxed, and friendly, lying down while stretching her paw out towards the assessor.
Reaction when softly spoken to:: Wizard leans her head forward and can be heard purring.
Reaction to cage door opening:: Wizard remains soft and relaxed.
Reaction to touch:: Wizard sniffs the assessor’s hand and accepts petting. She looks around while purring and leans in for chin rubs.
Reaction to being picked up:: Wizard remains soft during pick up and when held.
