Medical Summary DVM Intake Exam Estimated age: 4 weeks Microchip noted on Intake? n History : stray Subjective: eating on her own Observed Behavior – vocal, allows handling Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 220 R = wnl BCS 3/9 EENT: OU mild mucois dc, ears clean, no nasal discharge noted Oral Exam: decid teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: Ambulatory x 4, skin free of parasites, no masses noted,dirty coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: poor bcs, mild conjunctivitis Plan: erythromycin ou bid x 10d nutrical Prognosis: good SURGERY: Temporary waiver due to age
